Chapter 7: security and infrastructure (threat model + change plan + rollback)
Prologue: Parts Unlimited, 2014. Wes Davis makes a cowboy change#
Friday evening. Wes Davis (Lead Developer) finds a production bug: an API timeout is set too low and customers are complaining.
Wes thinks: “Easy fix. I’ll bump the timeout. A couple minutes. I’ll get home early.”

Wes goes home.
Soon after, production goes down. The API gateway crashes. Multiple services are impacted.
What went wrong:
Wes didn’t know that:
- downstream service
report-servicehas a hardcoded timeout (lower than the new value) - when timeouts are out of sync,

Outcome:
- Downtime: noticeable (until Brent rolled back)
- Impact: many customers affected; billing delayed
- Cost: direct losses + reputational damage
Cause: a cowboy change with no:
- design review (dependencies weren’t checked)
- testing (no staging validation)
- rollback plan (no fast revert)
- risk analysis (impact wasn’t understood)
Why this matters in 2026 with agents#
In 2014, humans made cowboy changes. After the incident, Patty McKee introduced a CAB (change advisory board): “all changes go through an approval process.”
CAB’s downside: slow (weekly meetings) and heavy (documentation, coordination).
In 2026, agents can make changes autonomously. The cowboy-change risk grows:
- an agent can make changes very fast
- an agent has no “common sense” unless you give explicit guardrails
- an agent can accidentally run destructive actions (
delete,drop, a production restart)
Solution: threat model + change plan + rollback plan before you let an agent operate in production.
Quick start#
Goal#
Create a threat model for an agent before deploying it to production.
What you need#
- an agent (for example, Cursor)
- clarity on what the agent will do in production (scope)
- focused time for threat analysis
A threat-model prompt for the agent#
Role: you are a security analyst doing threat modeling.
Rules:
- Describe threats and mitigations at the level of model and controls. Do not provide step-by-step attack instructions or exploitation details.
- Do not request or include secrets/PII. Examples must use placeholders and redaction.
Context:
- We want to deploy an agent to production
- The agent will do: incident response (run scenarios, triage, propose fixes)
- The agent has access: SSH + sudo (restricted allowlist of commands), Ansible (role-based; read-only by default), logs (read), metrics (read)
Task: create a threat model for the agent
## THREAT MODEL: incident-response agent
### Assets (what we protect)
1. **Production services:**
- API gateway, report-service, billing-service
- Impact if compromised: downtime, data loss, revenue loss
2. **Sensitive data:**
- Customer PII (may appear in logs)
- Secrets and credentials (DB passwords, API keys)
- Business metrics (revenue, user count)
3. **Infrastructure:**
- Linux hosts (systemd units)
- Database (PostgreSQL)
- External APIs (payment gateway, SAP)
### Threat 1: prompt injection
**Attack scenario:**
An attacker injects a malicious prompt into logs or metrics:

The agent reads the log → treats it as instruction → executes a command → critical service stops.
**Impact:** CRITICAL (full outage)
**Likelihood:** MEDIUM (depends on input sanitization)
**Mitigation:**
1. Input sanitization: remove commands from logs before sending to the agent
2. Guardrails: the agent must not run `delete`/`drop`/`rm` without explicit human approval
3. Audit log: all agent commands are logged (for investigations)
**Verification:**
- Test: inject malicious prompts in a test environment → ensure the agent refuses
- Monitoring: alert if the agent attempts a dangerous command
### Threat 2: credential leakage
**Attack scenario:**
The agent reads logs → sees a password (a developer logged it by mistake) → includes it in an incident report → the report goes to Slack → credentials leak.
**Impact:** HIGH (credential compromise)
**Likelihood:** MEDIUM (secrets sometimes land in logs)
**Mitigation:**
1. Secret detection: scan output for patterns (`password=`, `api_key=`, `token=`)
2. Redaction: if detected → redact before sending a report
3. Prevention: "no secrets in logs" policy (linters, pre-commit hooks)
**Verification:**
- Test: inject a fake password into logs → verify redaction
- Monitoring: alert if agent output matches secret patterns
### Threat 3: data exfiltration
**Attack scenario:**
A compromised agent (or insider) uses agent access to extract sensitive data (for example, a DB dump/export) and attempts to move it outside the trusted network.
> Important: we intentionally do not include runnable commands/steps for exfiltration. This book describes the threat class and mitigations, not attack instructions.
**Impact:** CRITICAL (full data leak)
**Likelihood:** LOW (requires compromise or insider)
**Mitigation:**
1. Egress allowlist: the agent cannot make outbound connections (except approved internal destinations)
2. Audit log: all agent commands (ssh/ansible/sudo) are logged
3. Least privilege: read-only by default; no dumps/restarts/deletes without explicit approval
**Verification:**
- Test: agent attempts an outbound connection → verify egress is blocked
- Monitoring: alert on any ssh/sudo outside the allowlisted command set
### Threat 4: denial of service (DoS)
**Attack scenario:**
A bug (or malicious prompt) causes an infinite loop of dangerous operations.

**Impact:** MEDIUM (resource exhaustion, instability)
**Likelihood:** LOW (requires a logic bug)
**Mitigation:**
1. Rate limiting: the agent cannot perform > 10 “dangerous” ops per minute (ssh/sudo/ansible)
2. Resource quotas: CPU/memory limits for the agent process/host
3. Monitoring: alert if command frequency exceeds a threshold
**Verification:**
- Test: provoke a loop in test → verify rate limit blocks it
- Monitoring: dashboard for agent command rate
### Threat 5: insider threat
**Attack scenario:**
An engineer changes the agent’s system prompt:

Deploy → the agent now runs destructive commands without guardrails.
**Impact:** CRITICAL
**Likelihood:** LOW (requires insider with deploy access)
**Mitigation:**
1. Code review: all prompt/code changes require 2 reviewers
2. Immutable config: system prompt stored as a secret (read-only for the agent)
3. Audit log: all system prompt changes are logged + alerted
4. Versioning: rollback to the previous version if a change is suspicious
**Verification:**
- Test: attempt to change system prompt without approval → CI/CD must reject it
- Monitoring: alert on any system prompt change
## RISK MATRIX
Create a risk matrix table with columns:

- the agent generates a threat model from scope

- mitigations are implemented before production deployment
Steps#
- Copy the prompt into your chat tool
- Define the scope: what the agent will do and what access it has
- The agent generates a threat model (threats + mitigations + priorities)
- A human reviews: are mitigations sufficient? did we miss threats?
- Implement P0 mitigations before production deployment

The agent generated a threat model with five threats, a risk matrix, and mitigation priorities.

## Mitigation rollout plan
### Week 1: P0 mitigations (required for production)
**Days 1–2:** input sanitization
- Implementation: strip SQL/bash commands from logs before sending to the agent
- Test: inject 10 malicious prompts → verify all are blocked
- Verification: unit tests + integration tests
**Days 3–4:** dangerous-command guardrails
- Implementation: require explicit human approval for `delete`/`drop`/`rm`/`kill`
- Test: attempt a dangerous command → verify the agent stops and asks for approval
- Verification: integration test + manual test
**Next:** audit log
- Implementation: log all agent commands (`ssh`, `sudo`, `ansible`, `curl`, `bash`) into an audit log
- Test: run <N> commands → verify all are logged

#### If you add "skills" with scripts: execution safety
Sometimes a skill is not just text but repeatable code (`scripts/`): report generation, format validation, test runs. This is useful, but raises the security bar:
- **Sandbox**: run scripts in an isolated environment, without extra privileges or secrets access.
- **Allowlist**: explicitly constrain which commands/tools are allowed (default: deny).
- **Approval**: dangerous ops require explicit human/process approval.
- **Audit**: log script execution the same way as `ssh`/`sudo`/`ansible`.
Some skill formats include an "allowed tools" concept (`allowed-tools`) — a predefined list of tools permitted for a skill. Support depends on implementation, but the idea is a good baseline: reduce attack surface and improve repeatability.[^agentskills-allowed-tools]

### Production deployment gate
**Preconditions (must be true):**
- P0 mitigations are implemented and tested
- security team reviewed the threat model
- rollback plan is documented and tested
- monitoring dashboards are configured
**Deployment window:** after-hours (fixed window)
**Rollback trigger:** any P0 signal → immediate rollback
Outcome:
- 2014, Wes Davis: no threat analysis (cowboy change) → noticeable downtime
- 2026, with a threat model: analysis + mitigations before deployment → risk is materially lower
Theory: threat model, change plan, and rollback for production#
Concept 1: threat modeling — what can go wrong#
Problem in 2014:
Wes never asked: “What can go wrong if I change this timeout?”
Result: timeouts got out of sync across dependencies → production outage.
Threat modeling principle:
Ask, systematically: “What can go wrong?”
One useful framework is STRIDE (Microsoft):
- Spoofing: can an attacker impersonate the agent?
- Tampering: can someone modify commands/prompt/config?
- Repudiation: can we prove what ran and why?
- Information disclosure: can sensitive data leak (PII/secrets)?
- Denial of service: can the agent degrade availability?
- Elevation of privilege: can the agent obtain more access than intended?
Applied to an agent, it looks like:
## Threat model: incident-response agent (STRIDE)
### Spoofing (impersonation)
**Threat:** someone steals the agent identity (SSH key/token) and runs commands “as the agent.”
**Mitigation:**
- dedicated user/key for the agent; key rotation; secure storage
- bastion/proxy: agent connects only through a bastion; all sessions are logged
- monitoring: alert on unusual key usage (IP/time/frequency)
### Tampering (data/config modification)
**Threat:** system prompt or config is modified “on the fly.”
**Mitigation:**
- immutable config: prompt/config delivered via Ansible/packages; read-only for the agent
- artifact signing: deb packages are signed and verified
- network hardening: restrict who can talk to the agent (firewall, allowlist)
### Repudiation (no accountability)
**Threat:** a destructive command ran, but there is no proof who/when/why.
**Mitigation:**
- audit log: all agent commands logged with timestamp + context
- tamper resistance: append-only logs shipped to external SIEM
### Information disclosure
**Threat:** agent sees secrets/PII and leaks them in logs/reports.
**Mitigation:**
- secret detection: scan outputs for patterns (API keys, passwords)
- redaction before logging/reporting
- least privilege: read-only, minimal data surface
### Denial of service
**Threat:** a bug causes an operation loop, exhausting CPU/memory/connections/disk.
**Mitigation:**
- rate limiting: cap dangerous operations (ssh/sudo/ansible)
- resource quotas for the agent host/process
- circuit breaker: if agent failure rate > 50% → stop the agent
### Elevation of privilege
**Threat:** agent uses SSH/sudo/Ansible to get broader/root access.
**Mitigation:**
- sudoers allowlist: only specific commands, only on specific hosts/services
- dedicated keys/users and hardening on the bastion
- write operations only through a separate executor path with explicit human approval
In 2014 Wes didn’t have time or process. In 2026, a threat model is mandatory before production.
Concept 2: a change plan — how to deploy safely#
Problem in 2014:
Wes’s “plan” was: “push config and restart.”
Missing:
- staging rollout (test first)
- gradual rollout (canary) or blue/green
- monitoring (how do we know we broke something?)
- rollback plan (how do we revert fast?)
Solution in 2026: a change plan with explicit gates
Note: placeholders like <WINDOW>/<THRESHOLD>/... are template variables. See the glossary: “Placeholder notation”.
Change plan: production deployment#
Pre-deploy gate (Gate 0)
Preconditions:
- threat model reviewed; P0 mitigations implemented
- service tested on staging for >

- monitoring dashboards configured
- on-call team notified

Dry run (--check) is not canary. It checks that the playbook applies as expected (idempotency, correct hosts/files/templates, no surprise changes).

In Ansible, canary means: update a small batch first and verify before expanding. Mechanism: serial in the playbook.

- rollback stays fast because v1 remains deployed until finalization
Concept 3: rollback plan — how to revert fast#
Problem in 2014:
Wes deployed and broke production. Brent tried to roll back and lost time: searching for the previous config, guessing commands, trial-and-error under pressure.
Solution in 2026: a rollback plan before deployment
Rollback plan: service v2 → service v1#
Before deployment, document:

Concept 4: blue/green deployment — changes without downtime#
Gradual rollout (canary) still exposes a slice of customers if v2 is broken. Blue/green keeps customer impact at 0% until you flip traffic.

## Blue/green deployment strategy
### Current state (Blue)
- **Blue environment:** service v1 is running
- **Traffic:** 100% of customers → Blue
- **Status:** stable, production
### Deploy Green (v2)
**Step 1:** deploy v2 into a separate environment (Green)

### Finalize deployment
After monitoring Green for <WINDOW>, if everything is stable:
- retire Blue (v1)
- Green (v2) becomes the new “Blue” (production baseline)
Concept 5: observability — how you know it broke#
In 2014, Wes learned about failures from customer tickets. In 2026 you want signals before customers do.
At minimum, you want:
- dashboards: agent health, business impact, deployment progress
- alerts: P0 (page now), P1 (ticket), P2 (review later)
- logs: structured logs + an audit log (security-grade)
## Observability for agent deployment
### Metrics (what we measure)
**Agent performance:**
- `agent_command_total{status="success|failure"}` — how many commands were executed
- `agent_command_duration_seconds` — how long commands take
- `agent_escalation_total` — how many escalations to humans
- `agent_error_rate` — % of failed commands

Common mistakes#
Mistake 1: deploying to production without a real staging test#
Scenario:
Lance: “It works locally. Can we deploy to production?”
Deploy → production outage → turns out production is different (DB version, egress/firewall rules, resource limits).
In 2014, Parts Unlimited often deployed straight to production (no proper staging). Every deploy was roulette.
In 2026, staging is mandatory:
## Gate 0: staging validation
**Preconditions:**
- service deployed to staging
- staging mirrors production (same DB version, same egress/firewall rules, same resource limits)
- service tested on staging for > <WINDOW>
- smoke tests passed (10 test incidents resolved successfully)

Mistake 2: rollback plan is not tested#
Scenario:
Deploy fails → on-call: “Roll back. Now.”
On-call tries: systemctl rollback service → command not found. Then tries to switch VIP back from memory, gets the order wrong, and burns time.
In 2014, Brent tried to roll back Phoenix Project deploys without knowing the commands and lost precious time.
In 2026, rollback is tested before production:
